Helping people to help you (Handy history chart)

In our experience, it is not uncommon for people trying to help you not to know what medicines you have had in the past, how well you did on it and what you thought. Medicine names aren't always easy to remember. This can be even more common if you go to a new area, see a new doctor or nurse or therapist, or go on holiday.

One way you can help others to help you is to use one of our "handy history" charts. You can open this here as a Word file. You can then save it to your computer. You can print it out and fill it in. Or, you can save it on your computer, fill it out and print it off when you need it, and maybe give a copy to people who may be trying to help you.

The Tees, Esk and Wear Valleys NHS Foundation Trust Pharmacy Service advises on and supports the safe and appropriate use of medicines within the trust. We do this by providing:

Complimenting our clinical activities our main hospital sites have dedicated on-site Lloyds pharmacies which supply and dispense medication for inpatients as well as service users who are being cared for by our community teams. An extended hours service in the evenings and at weekends is provided from designated Lloyds community pharmacies. This has enabled us to improve access to medicines.
