Get the best from medicines

Helping people to help you (Handy history chart)

In our experience, it is not uncommon for people trying to help you not to know what medicines you have had in the past, how well you did on it and what you thought. Medicine names aren't always easy to remember. This can be even more common if you go to a new area, see a new doctor or nurse or therapist, or go on holiday.

One way you can help others to help you is to use one of our "handy history" charts. You can open this here as a Word file. You can then save it to your computer. You can print it out and fill it in. Or, you can save it on your computer, fill it out and print it off when you need it, and maybe give a copy to people who may be trying to help you.

Open the "handy history" here as a pdf.

The Mental Health Pharmacy team within the Worcestershire Health and Care NHS Trust provide information and education about medicines to staff, service users and carers and work towards promoting best practice with their use.
